Who Is Eligible for Y-Haven?

Eligibility for Substance Use Treatment & Transitional Housing

Y-Haven serves adults age 21 and older seeking treatment for drug or alcohol addiction, including people who may also be experiencing homelessness, mental health challenges, or difficulties returning to the community after incarceration.

To be considered for admission, applicants generally must:

  • Be at least 21 years old.
  • Be seeking treatment for a substance use disorder.
  • Be medically and psychiatrically stable enough to participate in the program.
  • Be able to manage everyday activities such as bathing, dressing, cooking, and maintaining their living space.
  • Be able to live in a shared, supportive residential environment.
  • Participate in a pre-admission screening to determine eligibility and appropriate care.

Y-Haven also welcomes individuals with co-occurring substance use and mental health conditions, as well as individuals receiving approved medication-assisted treatment.

How to Apply or Make a Referral

Getting Started with Y-Haven Treatment

Y-Haven accepts inquiries and referrals from people seeking treatment for themselves, as well as from individuals and organizations helping someone access services.

Step 1: Contact the Intake Team

Call 216-431-2018, ext. 3452, or email intake@clevelandymca.org to discuss available services and begin the referral process.

Step 2: Complete an Intake Screening

The intake team will review eligibility requirements and assess the individual's needs to determine whether Y-Haven may be an appropriate fit and what level of care may be suitable.

Step 3: Review Admission Arrangements

If the applicant is eligible, the intake team will explain admission arrangements, program expectations, and what to bring.

Seeking Help While Experiencing Homelessness?

If you are currently experiencing homelessness, Y-Haven's referral materials instruct individuals to contact Cuyahoga County Coordinated Entry before seeking admission.
