Registration and Enrollment Forms

Registration must be received by 5:00pm on the Wednesday prior to the week of service. 

Each child must have all required registration and enrollment forms listed below completed in full and submitted prior to your child's first day in the program.

Full-time is 4-5 days per week
AM only care $38/week
PM only care $58/week
Both AM and PM care $85/week
Part-time is 1-3 days per week
AM only care $28/week
PM only care $48/week

County Voucher Participants

Please contact Portage County Department of Job and Family Services, 449 South Meridian Street - 2nd floor, Ravenna, Ohio 44266, (330) 297-3750, to obtain voucher approval for the center listed above prior to registering your child in the program.  The center is licensed as Aurora YMCA #105068.
