Sheffield Lake YMCA Summer Camp

When: June 11-August 17, 2012 (Programs will be closed on July 4th)

             Monday-Friday 7 am -6 pm

Where: Tennyson Elementary

               555 Kenilworth Rd., Sheffield Lake, Ohio 44054

Ages:*5-12 (*must have completed kindergarten)

Cost: $130 per week (full-time 4-5 days)

           $95 per week (part-time 1-3 days)

Registrations for the first week of camp must be received at the Payment Registrar office by noon on June 6, 2012.

Complete one registration form for

each child and one enrollment packet for each child and return to the Payment Registrar Office

Once your registration is received and processed, you will receive an email confirmation. Then you must print and complete the enrollment packet listed

on the right hand side of this page and

return it to the Payment Registrar Office

one week prior to you child's first day in the program.

Registrations are accepted on a first-

come, first-serve basis. You must complete all areas on the form to avoid

a delay in processing your registration.

Registrations will be time stamped in the

order they are received. Completion of the registration form does not guarantee space availability.

REGISTRATION FEES

A $25 per week/per child non-refundable deposit is required in advance for each week your child will be attending. This deposit will be deducted from your weekly program fee noted above. Deposits will be deducted from you account when registration is processed.

An annual membership fee of $25 per family is required for those not currently a YMCA member or YMCA program member.

Become a full facility member when registering for summer day camp and pay a zero joiner fee - a $100 savings.

PUBLICLY FUNDED CHILD CARE PARTICIPANTS

Contact Lorain County Department of Job and Family Services 42485 North Ridge Road, Elyria, OH 44035, (440) 323-5726 to obtain approval for the center prior to registering your child in the program.

The center is licensed as YMCA Tennyson Elementary License #308129.

FINANCIAL ASSISTANCE

The YMCA has financial assistance available to those who demonstrate need on a first come first serve basis. Financial assistance forms are available (here) or at any YMCA branch.

Field Trip: Monday, Wednesday, Friday (subject to change) (Calendar)

Swimming:  Wednesday and Friday at the French Creek YMCA

(please pack your child's swimsuit and towel)

Meals:  Breakfast and afternoon snack provided. Children must bring a nutritious packed lunch each day.

What to Bring

Children should wear comfortable shorts, YMCA camp t-shirts, socks and tennis shoes. For safety purposes, please do not send your child in shoes that slide off of their feet. 

Children should come with a non-refrigerated sack lunch and drink, water bottle, swim suit, towel and sunscreen each day. All items should be labeled with the child’s first and last name and placed in a sturdy bag or back pack. 

Please, no toys from home, cell phones or money. The YMCA is not responsible for lost or stolen articles.
